About the Program

The Children’s Long‑Term Support (CLTS) Program is a Wisconsin Medicaid Home and Community‑Based Services (HCBS) waiver that helps children and youth with significant limitations in daily activities remain safely in their homes and communities. CLTS provides individualized supports to help families meet their child’s needs and avoid institutional care.

Eligibility is determined through the statewide Children’s Functional Screen, and CLTS funding is used only after other available resources have been explored, as it is considered funding of last resort.
