.jpg) The Clerk of Courts is an authorized US Passport acceptance agency. Passports can be processed for travel 6 weeks or greater from the time of application. The only trusted website to print your application from is Travel.state.gov. if you have to pay for the application, the website is a scam. Applications are FREE.
THE FOLLOWING IS NECESSARY TO OTAIN A PASSPORT BOOK and/or CARD:
- Certified Copy of your State issued Birth Certificate.
- 1 Picture (2” x 2” on white or off white background.) Photos may be obtained at Walmart, Walgreens or Clerk of Court Office.
- A Check or Money order for each applicant, made payable to: US DEPT. OF STATE. PASSPORT BOOK: $130.00 for adult--$100.00 for minors 15 years old or younger.
PASSPORT CARD: $30.00 for adult--$15.00 for minors 15 years old or younger. Passport card is valid for land/sea travel between the U.S. and Canada, Mexico, Bermuda & the Caribbean ONLY - .$35.00 Execution Fee payable to the Shawano Co. Clerk of Courts.
- Photo Identification-Valid Driver’s License, Certificate of Naturalization, Official U.S. Military ID, Tribal ID
- Birth Date and Birth Place of both parents and spouse (if applicable).
- Name, address & telephone number of someone (not traveling with you) to notify in case of emergency.
- Social Security Number
Please allow a minimum of 30 minutes for the completion of you passport application. Applications accepted on a walk in basis from 8AM to 3PM Monday – Friday, the office is closed on holidays.Please note: Do NOT sign the application until you are in front of an authorized passport agent. Thank You!
ALL APPLICANTS AND PARENTS (if applicant is a minor) NEED TO APPEAR IN PERSON AT THE TIME OF APPLICATION.
EXPEDITED SERVIES: if you need a passport within 6 weeks, an additional $60.00 expedite
fee per passport is assessed.
There is an option of Express Mailing the application to the Passport Agency and/or receiving the
passport from the Agency. Please keep in mind Passport Cards cannot be returned via Express Mail.

REQUIREMENT FOR CHILDREN
As of February 1, 2008, Congress passed a new law (code of Federal Regulations, Title 22, Section
51.28). This requires the signatures of BOTH parents on U.S. Passport application for children under age 16.
Applicants ages 16 and 17 only need to appear with one parent.
