General Information

Shawano County certifies homes or apartments of interested parties who want to provide adult care services to any of the following target groups: chronic mentally ill, developmental disabled, frail elderly or physically disabled. The home or apartment must meet the state regulations and standards. The Department of Human Services seeks qualified families to provide cares to those who cannot live alone, yet wish to continue living in a family environment. The requirements to become a certified adult family home include the following:
  • Willingness to take someone into their home that meets the criteria of a vulnerable target group and provide them with a loving a nurturing home
  • Must pass a background check
  • Be at least 18 years or older
  • Be willing to follow standards set forth by both the county and state mandates
  • Understand and respect others right of choice, confidentiality and need to be an active member of their community
  • On-going connection and communication with the participant’s support team
  • Annual training to meet the needs of the participant
The process to be certified to be an adult family home will include the following:
  • Application process, personal reference and recommendations
  • Visual inspection of the home to ensure it meets state guidelines
