General Information

Our offices are located in both the Fellman Center (for individuals with physical disabilities and the frail and elderly) and Lakeland Center (for individuals with development disabilities and chronic mental health issues).

Long Term Care Services - Adults
Long term care services are provided to adults who require a full array of services and supports, due to long term and chronic disabilities, in order to live within the community successfully in non-institutional settings. Adult long term services are primarily funded through the Medical Assistance program. An individual’s eligibility is determined by the completion of the State approved Wisconsin Adult Long Term Functional Screen. The type of services and support is based on the individual’s assessed needs and person centered planning. The person center planning is a team approach to maximize independence and the individual’s involvement in his or her community.

Case Management – Adults
Case management and service coordination is the provision of service to locate, manage, coordinate and monitor program services, additional services and informal community supports provided to eligible individuals and to assure that services are provided in accordance with program requirements and provided in accordance with program requirements.

Residential Services – Adults
Linkage to the coordination of residential services based on an individual’s abilities and needs, including supported home care, daily living skills, or household services.
